Signing up is easy-- just follow the steps below.
If you would like to complete an application and pay online please follow steps 1-4 below.
Practitioner - $75.00 (Annually)
Friends of the NDRC - $25.00 (Annually)
Step 1: Click the button below to obtain a Membership Application
Step 2: Complete the application and save the document to your computer along with any required documents you wish to submit with your application.
Step 3: Fill in the form below. You will be allowed to submit your completed application with documents by clicking "Choose File" Once you've selected the documents you wish to submit, click SUBMIT.
Step 4: Select your membership level and provide payment.
